By using this website or submitting the appointment request form, you agree to the practices described here. This policy covers the website only. The medical records created when you become a patient are protected health information governed by federal and state healthcare privacy law, including HIPAA, and are handled under the Notice of Privacy Practices you receive at the clinic, not under this policy.
Information you give us
When you submit the appointment request form we collect what you type: your name, phone number, email address, the type of care you are asking about, your preferred language and, if you write one, your message. We also record the fact that you agreed to receive text messages, together with the exact consent wording shown to you, the language it was shown in and the time of submission, so that your consent can be verified later.
Information collected automatically
Like most websites, our hosting provider keeps standard server logs (such as your IP address, browser type, the pages you request and the time of the request) for security, troubleshooting and to keep the site running. The form is protected by Cloudflare Turnstile, which analyzes technical signals from your browser to tell people from automated bots and may set a cookie of its own for that purpose. We do not run analytics or advertising trackers, and this website sets no cookies of its own.
Do not send medical information through this website
The appointment form is a public web form, not a secure medical record, and it is not intended to receive protected health information. Please do not include medical history, diagnoses, symptoms in detail, test results, insurance policy numbers or any other clinical or financial information in your message. Any such information you choose to send is provided at your own risk and will be handled only to the extent needed to respond to your request. Bring that information to your appointment instead.
How we use your information
We use the information you provide to respond to your request, contact you to schedule and confirm appointments, communicate with you about your care request, keep a record of your contact and consent, protect the website and our systems against abuse and fraud, and comply with legal obligations. We do not use it for any purpose that is incompatible with these.
Text messages (SMS)
By entering your phone number and agreeing to the SMS statement on the form, you consent to receive text messages from PI Medical Doctor at that number regarding your appointment request, appointment reminders and follow-up on your care. Message frequency varies. Message and data rates may apply. Reply STOP or UNSUBSCRIBE at any time to opt out, or HELP for assistance; you can also contact us using the details at the end of this policy. Your submission is processed by service providers that make the website work and that act on our instructions and for no other purpose: Vercel hosts the site, Cloudflare Turnstile verifies that the sender is a person, Resend delivers the notification email to our team, and our appointment and messaging system stores the request so the team can follow up and send the texts you agreed to. We may also disclose information when required by law, subpoena or court order; to protect the rights, property or safety of PI Medical Doctor, our patients, our staff or others; to investigate fraud or abuse of the website; or, if the clinic is ever sold, merged or reorganized, to the successor, who will remain bound by this policy.
How long we keep it
We keep the information you submit for as long as needed to respond to your request, schedule your care and keep a record of the contact and of your consent, and for as long as the law or our legitimate record-keeping needs require. You may ask us to delete it at any time and we will, unless we are required or permitted by law to keep it.
Security
We use reasonable administrative, technical and organizational safeguards to protect the information you send, including encrypted (HTTPS) transmission, bot protection on the form and access limited to the staff who need it. No method of transmission over the internet or of electronic storage is completely secure, so we cannot guarantee absolute security. If a security incident affecting your information ever occurs, we will notify you as required by applicable law.
Your choices and rights
You may contact us at any time to see, correct or delete the information you sent through this website, to withdraw your consent to text messages, or to ask us to stop contacting you.
